Community Health Education Specialist

Essential Duties and Responsibilities:

  • Health Promotion & Education
    • Develop a comprehensive community health promotion & health education outreach plan to reach the medically underserved population with special emphasis on special populations (homeless, migrant & seasonal agricultural workers, public housing residents).
    • Develop and deliver health educational presentations, workshops and group sessions to target populations in the community.
    • Conduct health education in-person and/or virtually with culturally and linguistically appropriate content.
    • Lead curriculum-based programming for patients and community members.
    • Conduct chronic disease prevention, oral health, HIV/AIDS, COVID education during clinical hours and non-clinical hours to reach target populations.
    • Assess needs of the community, coordinate and facilitate health educational events.
    • Establish vaccine campaigns (immunizations, flu, COVID, etc.) and other services while working directly with Manager to organize events.
    • Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as, community education, learning webinars, and training opportunities.
    • Take part in community activities designed to allow target populations to access prevention information.
    • Serve as an ongoing resource for patients and community members in relation to their health goals.

  • Community Engagement
    • Collect and analyze data from community stakeholders to help determine programming.
    • Research subjects in advance to deliver material with confidence.
    • Offer a wide range of activities to reach different learning styles when facilitating educational sessions, activities, programming.
    • Conduct research and develop programming to address the needs of the health center and targeted community.
    • Maintain databases, mailing lists, community contacts and other information to ensure cooperative working relationships with agencies and community partners interested in health education.
    • Document activities and records information such as presentations conducted, persons assisted, and health assessments completed.
    • Collaborate with health specialists and civic groups to assess health needs and goals for meeting those needs.
    • Conduct activities to identify interest and needs (ex. public health surveys).

  • Other Duties & Responsibilities
    • Ensure confidentiality and HIPAA compliance at all times.
    • Keep other care team members informed when situations occur that disrupts timely patient flow through site.
    • Adhere to patient care needs and the core values of Premier Community HealthCare to a provide best-in-class patient experience.
    • Work towards attaining department goals and drive Quality Improvement/Quality Assurance (QI/QA).
    • Other duties as assigned.

Knowledge, Skills and Abilities:

  • Ability to work with culturally and linguistically diverse populations.
  • Excellent presentation, communication and facilitation skills.
  • Well versed in specific health education and/or health industry trends.
  • Ability to coordinate with multiple disciplines within the health center and community organizations.
  • Proficiency in Microsoft Office applications and virtual applications such as (Zoom, Google Meets, MS Teams, etc.)
  • Exceptional patient engagement and patient experience skills.
  • Ability to communicate effectively, verbally and in writing with multiple levels within the organization.
  • Excellent grammar, spelling and interpersonal skills.
  • Available for extended hours, occasional evenings and weekends.


  • Degree in nursing education, nutrition, health education, community health, public health, health sciences, human services, or other related field OR equivalent education and experience
  • Two (2) years of direct health education experience or community engagement experience
  • Bilingual preferred
  • Health Educator certification preferred
  • Valid driver's license and reliable transportation
  • Ability to drive vans/larger vehicles
