SUMMARY:

The Quality Care Services Advance Practice Provider (Nurse Practitioner or Physician Assistant) is integral part of the Premier Community Health care team. The APP works in collaboration with the QCS Team by conducting in-person or virtual assessments to evaluate the current health status, gaps in care, potential health risks and care management opportunities of patients, with a focus on driving better clinical outcomes.

PROFESSIONAL KNOWLEDGE, SKILLS AND ABILITES:

  1. Master's degree from an accredited Nurse Practitioner or Physician Assistant Program
  2. Current Florida NP certification and RN license or PA license
  3. Prefer a minimum of three (3) years' experience as a Nurse Practitioner/ Physician Assistant
  4. Current Basic Life Support (BLS) certification.
  5. Knowledge of CMS Regulations and NCQA HEDIS Guidelines;
  6. Knowledge of Evidence-based Clinical Practice Standards: American Diabetes Association (ADA), American College of Cardiology (ACC), American Heart Association (AHA)Familiarity with ICD-9, ICD-10 and CPT-4 coding practices

Advanced Practitioner Registered Nurse (APRN) / Physician Assistant (PA) Family


ESSENTIAL DUTIES AND RESPONSIBILITIES includes the following, other duties may be assigned

  • Complete comprehensive, accurate and thorough review of the assigned member population, including timely completion and submission of all required encounter documentation (paper or electronic).
  • Performs complete history & physicals and documents in the medical record.
  • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
  • Ensure that all pertinent and active medical conditions are documented in the medical record in a manner compliant with CMS regulations and health plan requirements
  • Support the practitioner/patient relationship and ensure timely and adequate communication, documentation of assessment findings, recommendations, need for additional services, emergency services required if necessary and need for follow up and timelines for follow up to primary care provider (PCP) and health plan as required;
  • Make general recommendations to members intended to improve members' knowledge of their chronic condition(s), such as information concerning recommended testing.
  • Address and close identified gaps in care (disease-specific or preventive).
  • Recognize emergent or urgent situations requiring escalation and take appropriate action as specified in PCHG policies.
  • Perform, document and communicate results of Point of Care (POC) Testing.
  • After hour services.
    • Participate in rotation of on call assistance.
    • Advise patients of appropriate after hour care.
  • Ensure patient confidentiality and EMR security at all times.
  • Maintain patient information within HIPAA.
  • Keep other care team members informed when situations occur that disrupts timely patient flow through site.
  • Consistently participates in and actively adheres to patient care expectations to attain clinical goals set forth in the PCHG Health Care Plans and the Quality Improvement / Quality Assurance (QI/QA).
