SUMMARY: Serve as an integral part of the Patient Centered Medical Home by facilitating, monitoring and evaluating interdisciplinary care to Premier Community HealthCare Group (PCHG) patients. Responsible to assist patients in management of their needs, including but not limited to: medical and/or social, health education and care follow-up.

ESSENTIAL DUTIES AND RESPONSIBILITIES: includes the following, other duties may be assigned.

Uphold Clinical Standards of PCHG.

Adhere to Occupational Safety and Health Administration/Department of Health (OSHA, DOH) and PCHG Safety guidelines.

Abide by established protocols & performs only those functions for which employee is educationally prepared.

Function as Care Coordinator/Case Manager.

Act as advocate for an individual's care needs by identifying and communicating opportunities for care interventions, including identifying and addressing functional deficits and gaps in care.

Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with patients.

Identify members with the potential for high-risk complications and coordinate the appropriate supported self-care in conjunction with the member and the health care team.

Daily patient pre-visit preparation and documentation.

Daily "huddles" with care team members.

Perform processes for recalls and tracking using established patient management system.

Develop and implement targeted strategies to improve health, functional or quality of life outcomes, such as disease management or pharmacy management.

Monitor hospitalizations, emergency or urgent visits, institutional facility admissions and re-admissions to identify issues and implement follow up with the Patient Centered Medical Home for improved outcomes.

Establish and maintain professional working relations with referral sources, community resources and care providers.

Routinely assess and monitor patient's status, needs and progress; if progress is static or regressive, determine reason and proactively encourage appropriate adjustments to their plan of care, provider and /or services to promote better outcomes.

Implement patient teaching plans.

Coordinate and facilitate patient education groups.

Ensure patient confidentiality and EMR security at all times.

Maintain patient information within HIPAA guidelines.

Keep other care team members informed when situations occur that disrupts timely patient flow through site.

Consistently participates in and actively adheres to patient care expectations to attain clinical goals set forth in the PCHG Health Care Plans and the Quality Improvement / Quality Assurance (QI/QA) 

SUPERVISORY RESPONSIBILITIES: as deemed by licensure in the State of Florida.

GENERAL DEVELOPMENT:

PROFESSIONAL KNOWLEDGE, SKILLS AND ABILITES:

Current Florida licensure for Registered Nurse (R.N.) or Licensed Practical Nurse (L.P.N.) or Medical Assistant from an accredited school.

 

Current Basic Life Support (BLS) certification.

Current Continued Education Units (CEU).

Basic understanding of medical terminology and documentation.

Ability to use basic mathematical skills.

Able to preform repetitive tasks.

COMMUNICATIONS SKILLS:

Exceptional Customer Service skills.

Ability to communicate effectively, verbally and in writing.

Excellent grammar, spelling and interpersonal skills.
